This page explains what the forms on littleopticsny.com collect, where that information goes and who can see it. It describes the website only.
The eye exam and contact lens request forms ask for the patient’s name, date of birth, phone number and email address. You can also add your insurance provider, your member ID and photographs of your insurance card. The insurance fields are optional — you can leave them blank and bring the card to the visit instead.
The contact forms ask for your name, phone number, email address and your message. Please do not include medical details in them. For anything urgent about your eyes, call the office at 347-738-5317.
What you send is stored by Wix, the platform this website runs on, and appears in Little Optics’ own dashboard. Only the Little Optics team can open that dashboard. Nothing is sent to advertisers or sold.
If you upload a photograph of your card, the file is stored on Wix’s servers. The address of that file is long and not guessable, but it is not protected by a password: anyone who had the exact address could open the image. If you would rather not upload it, leave those fields empty and bring the card with you.
You can ask us to delete a request or a message you sent through this website. Call 347-738-5317 or write to littleopticsinc@gmail.com and tell us the name and the date you used.
This website does not add advertising or tracking cookies of its own. Wix, as the hosting platform, may set cookies it needs for the site to work and for its own statistics.
Note: this page describes how the website handles what you type into it. It is not the practice’s HIPAA Notice of Privacy Practices, which covers your medical records and is provided by Little Optics directly.
